Before we can understand this fully, we’d need to know the server settings and mods.
If they cannot, it is possible a golem accidently pushed a thrall off a high place while harvesting or moving. This is speculation, of course.

Yeah, it could be an accident.
But it happened before, I saw other reports of same situation.
Anyway, I just assumed it was a bug or so and I gave up of having my thrall back
