Bug Description:
After leaving the Jhebbal Sag Dungeon Midnight Groove we died several times beside that small island in sepermeru by drowning.
I don‘t think that we got killed by the cook because the Event log does only tell that i was killed, but not by who.
Even the spawn location suggested death bei drowning, because you cannot stand there. (Had to dive to loot myself).
It does not happen every time, but i noticed it probably happens when more than one person, at least two, leave at the same time.
Sadly i wasn‘t fast enough to screenshot, but i will try to submit later.

I confirm the problem, you need to prove nothing, it does happen but not all the times. My event log said once that I killed my self and another that I was killed by the werewolve I killed before exiting . But I must confess both of the times I left the area as soon I had killed and farmed the werewolve and the ghost was still speaking. I don’t know if that has something to do with it.
This happened to me as well but since I had a PS5, I loaded in time to get air. I appeared about 3-4 foundations North by Northwest of the tip of the cook island.
