I raided a guild who had a T2 Rift of Yog as a placeable item in a chest, how did they do this? As far as i knew, you could only get a T2 shrine from upgrading a T1.
Probably glitches/exploits/hacks. Make sure you wipe them off of your server.
When we could loot other players decayed/demolished buildings on PvE servers I found t2 or t3 alters in boxes on two occasions.
What I think happens is sometimes very rarely when an alter is being upgraded the process has a glitch and instead of upgrading the alter it stops leaving the alter in the current alters inventory. Usually, the game will upgrade the current alter and either everything is ok OR there is a problem with overlap/space available/npc or player in the way and the alter just despawns. Very very rarely I think it ends up in the alter inventory and then you can move it.
One year ago one time happened to me with a Set t1 altar: trying to upgrade it to t2 resulted in obtaining an item on the t1 altar wich was the t2 altar.
So I had to destroy the old altar and place the new one 
Never happened again so I think it’s an already fixed old bug.
