Hunting the last trophy "The towers gemstone"

I don’t know if the translation is correct. In german it’s called “Der Edelstein im Turm”.
Does this trophy requires something else or just discover all map markings?
I guess i discovered nearly all of them. Maybe i overlooked some or need to do anything else. Or maybe this last trophy is bugged?
Can someone please give me a hint?

The Gem In The Tower achievement is bugged at the moment. It should work as intended in update 2.5 which is currently on testlive.
