I have discovered all locations on the Siptah map and have not received the achievement. This includes the infamous Cracked Vault.

Bug Reproduction:

Suppose I could try restarting and doing it again, but ain’t no one got time for that

There is a new location with 3.0 in siptah. It is not on the interaktive map. Try isle of dusk (the dark brown isle) the most south west point

Yes, that is the aforementioned Cracked Vault you are alluding to, which I visited multiple times prior to posting this.

I believe you are missing ‘‘Scavenger Coast’’ in the coordinates O10. Perhaps after discovering that area the achievement will unlock for you.
