Have your character move slower and not hit as hard the closer they are to death.
It would be realistic, yes - but would it improve the gaming experience? Most of the game, including the whole damage and hitpoints mechanism, isn’t realistic, nor is it supposed to be.
Your suggestion would simply lead to wounded characters dying faster. Eithet they couldn’t finish off whatever enemy got them into such a state, or they’d be unable to reach safety if they tried to escape. Unless, of course, NPCs were similarly affected and started to do proportionally less damage as they get hit. It would require a lot of balancing to work properly, especially considering healing items that can be used during combat.
