Thrall dies without reason

Once a Thrall dies while User was climbing a building without reason in Event-Log.
Apotheken time Thrall dies while User was climbing with reason “killed by Black leagion” but there was noobe near the user.

Please provide a step-by-step process of how the bug can be reproduced. The more details you provide us with the easier it will be for us to find and fix the bug:
