Can someone explain what happened to my slave? Did the chair kill him?
Probally got killed
There are several possible reasons, from bugs to hacking. Hard to say without any further information. The “living settlements” system is a prime candidate. Apparently, it is still active on some servers.

The same thing happened to me, I had some minor damage to the structures and it appeared that I had been killed by slaves, an “unknown” player and an NPC.
