Bug Description:

“Tower of the Elephant” Trophy does not work

Bug Reproduction:

I tried to unlock this trophy with a friend. Both on our server and co-op with or without admin help it did not work. I even had the maximum of items in inventory and reached an overload of over 20K. Nevertheless, the player does not die under me.

It cannot be done in PvE, try switching your co-op game to PvP and give it a go once more.
