WTF?! I logged in dead, gear lost, thrall inventory gone

When I logged in today, after downloading the latest patch & DLC’s, I had died, all items in my inventory lost, my Relic Hunter thrall has nothing in her inventory (Godbreaker Armor gone!!!), and the thrall won’t follow, and can’t be placed!!!
