Everytime I die I spawn at different location in different dungeons

Basic Info:

Bug Description:

I was killed at my base that is located at f/4 on the map. when I respawn I am in a dungeon when i kill my character i then respawn in a completely different dungeon.

Bug Reproduction:

I died.

Regarding the situation at hand, does this happen every time you die regardless of your position? If so, could you provide us with a video of this occurring?
