Instant death spot - “Fell through the world”

Bug Description:

Twice I’ve died by “falling through the map” and lost a ton of farm. I’m just walking down a path or wherever I’m going and boom instantly dead with a message telling me that I fell through the world. The first spot was east of xalthar’s crossing when walking up the sand path. The other was at northern entrance of the under water passage west of the watcher of the passage landmark. Please fix this. I’ve lost a ton of stuff on two different occasions - days apart. We shouldn’t have to worry about stepping on the wrong tile and losing hours and hours of progress.

Bug Reproduction:

Just walk on the right spot and you’ll instantly die. Your items will not be able to be recovered. Your body cannot be looted and says it has nothing on it.

I have died in two other spots in the world as well. On exiled lands in Jhils roost running down the ramp to the final boss and in the sinkhole right before the tablet you learn dragonbone armor and weapons from. Same thing happened and I lost everything. It’s like their anti mesh was turned up a little too much in the last update. Because the spots where I died you could mesh ever so slightly into the landscape.
