Poison Applied To Weapons Icon not removed on depletion

Game mode: [ Single-player)]
Type of issue: [ Bug]

I applied the sand reaper poison to my sword. It worked fine but when the number of applications was up the icon remained on my sword.
Also happened for the purple potion against undead.

Please provide a step-by-step process of how the bug can be reproduced. The more details you provide us with the easier it will be for us to find and fix the bug:

  1. Apply poison see icon
  2. Hit things and watch them get poisoned
  3. Poison icon on weapon does not disappear
  4. Persists even after quite and reload for singleplayer
