My two-handed stone sword went invisible. Removing it from the hotbar and re-assigning it to the hotbar solved it. It still did damage it was just invisible.

Could not reproduce.

This happened to me after dying with some daggers equipped. Switching to another weapon and back fixed it. Did you also die with sword equipped?
