Sanctuary of the Serpent - fall through the mesh

When I’ve completed the Serpent Vault and went to the exit, instead of opening the door my character felt through the floor and dropped undermesh. Fortunately I’ve got some buff sigils on me so I managed to survive the fall damage. After a short flight, my char was dropped on the coast with low hp, you can see it on the map.
