I will go check it today and later i will respond. Note that my answer will come from ps4 platform.
Since it seems you’re on multiplayer, check on top of the cave if someone placed foundations or building/claiming blocks above the cave, if that’s the case then somebody is blocking the respawn of the mob.
That cave should be instanced, or at least above it should be a non buildable area.

Unfortunately my friend (for your server) the king is alive and killing 
, so no bug for barrow King. Both, server and home alone, the king spawns normally. Note please that i confirm his existence from ps4 console
