Can't get something out of shortcut wheel

Bug Description:

So it happens when I’m in combat. I’ll be attacking and then one of my items like for example, my shield will return to my inventory and my wheel puts a “XX_Unarmed Right” or “XX_Unarmed Left” and i cant get rid of it and put anything in that slot. The only way i can get rid of it is killing my character and it goes away until it happens again.

Bug Reproduction:

I tried removing and putting items in that slot to make it go away. I tried using it, to attack and my character stutters and doesnt move. I cant make it go away.
