Item XX_Elephant_dc_1 appeared after Upgrade an Elephant to Caravan Elephant.

That’s an old known issue. Sometimes, the pet’s attacks appear in their inventories when spawned in the world. It’s a fairly inoffensive one as it doesn’t affect the behavior of the pet in any way. Nonetheless, our team is aware of it and they’ll give it a look.
