Chaosmouth isn't making the Keystone

So I got all six items to make the keystone. I went to the Chaosmouth altar to make it. However when I put the items into the altar, the items simply disappeared leaving the altar empty without creating the keystone. This happened twice. What is going on here? How does this get fixed?
