I can’t understand how my thrall died, the system says “died by Darshan Warlock” How can this be?

Was the thrall following you?
Are you on a PVP server?
Maybe gas? It could also be Freya’s perk (bleed), if you were dragging that thrall on the leash (to WoP).
Maybe you hit it while being released from its cage. Did you get the revive window to pop-up? I don’t know if you opened a cage, but it seems that way. It’s a little tricky there. I’m just trying to figure out what toon died first.

No was following, and its a PVE server.
That happens.
When the game decides to randomly delete one of your followers, it may say they were killed by you or another clan mate.
This one frequently sees it on Siptah, where the thrall deletion bug doesn’t have a convenient spot in the highlands to teleport your property person to.
A day later the log may instead simply read “died” rather than falsely blaming you for it.

Was this a crafter thrall?
Alright. The thrall might have died by you, as in friendly-fire. I know that was isolated, so it is turned off on officials.
