My wife died near the Black hand ship base the first time, then she died again shortly after while trying to get back to her first corpse. After a little while I got on to help her get to her corpse. We got to her second death marker and she looted it but the marker for her first death wasn’t on the map. We searched for the body/loot but it was nowhere to be found. I’ve heard this is a fairly common bug that has been around since release but I don’t know how to reproduce it.
Please provide a step-by-step process of how the bug can be reproduced. The more details you provide us with the easier it will be for us to find and fix the bug:

I dont think its bug, more likely corpse just despawned, as bug were refered corpses not spawning at all, and it were solved, and even if it be bug try relogin, it usualy solve problems like that

This is behaviour we expect and I am sorry for your wife’s loss. The second death did wipe the loot from the first death. When running with another player, I usually stay with the first tombstone, making sure there is nothing that would get between them and their gear.
The alternate solution that I use while solo is replicating the gear and leaving it in a special box bear each spawn point. I can grab all and dress on the run. With legendary weapons, I get as close as possible with crafted ones, good enough.
Please help your wife through this traumatic experience !
Thank you for your response Jim. Unfortunately that glitch is a game breaker for her and my roommate. I will continue playing but it means that she won’t clear me getting the dlc. If y’all fix that bug or even have plans to remedy it in the future please let me know so I can get them back on board.

Tombstones despawn after a period of time to avoid cluttering (30 minutes in official servers). It is not a bug, but intended behavior. As per the death marker, it shows where you died last only.

I just double checked on Solo play…
You can have up to 3 tombstones on the map, and of course 3 corpse/cairn’s.
As soon as you die a 4th time the first tombstone vanishes of the map and it’s cairn turn’s in to a loot bag.
All subsequent deaths destroy the oldest tombstone/cairn.
If your quick enough (within 30 mins of the 1st death) you CAN loot all 3 cairns.
This was done to give an unarmoured player 3 chances to get their original kit back
I can’t speak to three tombstones, but this evening I was running with a friend, let’s just call him Cliff (for obvious reasons).
He died while trying to slide down a long drop, leaving his tomb with all his gear. Having his spawnpoint back at home base, he portalled to the sinkhole, whereupon an undead hyena ripped his nekkid bod to shreds. Second death, still no gear.
I stood at the obelisk to keep the hyenas at bay and back he came. Making it through this time, we proceeded to his first death marker and made the slide off the cliff without incident. Gear retrieved, two tombstones on the map, well less than a half hour.
I’m with @ChatterBox in calling this a bug, but reproduction would be iffy at best.
Please ask your wife to read this thread, perhaps it will change her mind. It’s a wonderful online/partnership experience.
That’s funny, I haven’t seen a Dark Souls server setting. Just kidding, but seriously I haven’t seen that in the server settings. Also I have heard that this has been going on since the release of the game so I have to assume it’s a bug in the core programming. If it is, I understand that it is a major pain to do a rewrite. With that said, my suggestion would be to copy the code, find the bug, fix it, then re-release the updated version. You may get a lot of backlash for it but it should be done for a couple of sound marketing reasons. The first reason is that without fixing the big bugs and focusing manpower on pumping out xpacs it gives the game a money grabbing feeling. The second reason is that by fixing the bugs it would give the player community a sense that the dev team really cares about their opinions and concerns which creates a loyal customer base, And lastly with a loyal customer base and good solid programming you will have a game that could compete heavily in the MMORPG arena. Heck if someone started a Kickstarter to make up for loss of profits while the game was offline I would pay into it.
@drachenfeles, could you point me to those settings? Even if in a .ini, it would be interesting to test. We may even prefer a single gravestone, just to punish our friends
LOL, for some reason in my solo game it is set to 10. I certainly did not do that.
@LucanDChance, since you are playing on an official server, you have no way of altering serversettings.ini, which contains the setting @drachenfeles just turned me on to.
You could, however, start a new game in solo/coop and invite your wife as a player through Steam (must be friends). There are a few restrictions, none too harsh, tether range being the most restrictive.
Find ServerSettings.ini and alter CorpsesPerPlayer to any number that makes her happy.
An alternative would be renting a server, then more than two can join. They are relatively cheap and as server admin for CE, do the same. Then you, better half and her BFF can play together without those restrictions.

@drachenfeles, I fully concur. I got my daughter a laptop and the game as a present, thinking we could spend some quality time together 1k miles apart. The rigamarole we had to go through with steam, other coop problems, 2 player limit, etc, drove her batty.
My issue with server hosting is opening ports and getting rather deep into the digital mechanics to allow multiplayer. Personally, I would rather throw money at that solution since it is rather cheap.
I suspect you are rather proficient and comfortable with hosting and surpass my knowledge /bow
