The bug one is The Silver Key and The Other Gods. I tried to kill the dragons (3skull boss) both in The Fractured Citadel and while during the Maelstorm with every tool: pick, hatchet, pickaxe and sickle but the achievement didn’t appear. tested with the last demon boss who gave the shining ??. the other achievement popped out when i did the tier 1 surge (the one with 10 Swirling Chaos needed)
