this one is a bug, not linked to a malicious action, please look at this thread Character death at server restart - PlayStation Discussion / PlayStation Bug Reports - Funcom Forums , toons sleeping in wolrd die randomly after a server restart (they can go through serveral server restarts, but randomly die (low probability but happens). it happens since around 3 months and happen also on pc server (official)
