When I kill the Cultist Leader for her access card I can’t pick up the card and the mission blanks out.
Blank Mission Tasks are a known issue our developers are currently investigating. A Game Master will be happy to assist you with this if you submit an in-game petition.

Type /petition in game to open that interface.
Did that this morning. Still waiting on a reply.

I have the same issue on the same mission (though tier 2). Game Masters have not responded and I’ve had an open petition for a week.
