Message of Thrall Death and Nothing Else

Had an issue where message that my thrall was killed. I was near my base. He is definitely gone. He’s no longer in my follower log; however, nothing of his death or a loot bad dropped in the event log. No body or anything. So I don’t know if he up and went to the rapture or what or phased through the earth or what.

-Proximity: max
-Days: 7
-Only check the thralls checkmark

this happened to me twice since last update, 1: berserker near mitra shrine a rhino hit me and i flew like a fkn mile away and inexplicable thrall death. 2: volcano base, frost giant hadda walk 10 steps to the maproom but disappeared and died instead.
