Attempted to summon body after death. Successfully intitiated the body summoning, ritual completed with no body being summoned. Attempted ritual several times including moving the summoning circle to outdoor location with no ceilings. Each time the ritual was initiated it completed with no warnings. But no body summoned. Looked for unrendered or invisible body with nothing found. Attempted reloading game. Ritual still initiated with no warnings and no body was returned. Lost a lot of high level gear, keys and resources.

PC, independent online server , no mods.
