Call an vertical elevator with a npc/pet following you can cause instant kill them. Only calling elevator when it is in high side position.
Please provide a step-by-step process of how the bug can be reproduced. The more details you provide us with the easier it will be for us to find and fix the bug:

If npc/pet is near from base of elevator system cause instant kill (only calling elevator, i dont say with weigth of the elevator)
Pets and Thralls try to teleport if you go to far away. So if you wanna use some elevator, simply set them on Guard and place them on the Elevator platform. Dont let them follow while using an elevator.
For me they work like intended. If you go/swim/climb to far away they teleport to your position (better than staying there and doing nothing anymore).
So if you are using an elevator they teleport right next to you like they allways do, but on a elevator platform is NO right next to you, so they falling down because there is no platform they can stay.
So i guess, it is up to the pet/slave owner to think first and for the pets/slaves, like in RL.
I wont go with my dog on the leash into an elevator and let him stay outside in the stairwell and start the elevator. That must go wrong.
