It’s a bug, so not your fault. There’s no need to change your character name.
It happens if you log in to the server and there is no character already spawned in for you, meaning someone killed your unconscious body.
It is fixed internally, and should be in the next testlive update.
Not really sure if there is a workaround for the issue right now. Beyond changing a value in the game.db, which you can only do if you have access to the hosting server.
If you do have access, or if anyone else who has access wants a workaround:
- shut down the server
- open game.db with an sqlite browser.
- find the characters table
- find your character
- change the IsAlive value to 1
- restart the server
