Achievement for entering Sinner's Refuge only received when I leave

Bug Description:

I only get the achievement for entering Sinner’s Refuge just as I am leaving the cave. I noticed this several times now on a different low-level character on multiple servers. I am wondering if the definition of Sinner’s Refuge entrance got turned around somehow, or maybe, Sinner’s Refuge is now bigger on the outside than it is on the inside??

Please provide a step-by-step process of how the bug can be reproduced. Please be as detailed as possible; the more details, the easier it will be for us to find and fix the bug:

  1. Enter Sinner’s Refuge,
  2. Achievement is not rewarded right away.
  3. Exit Sinner’s Refuge,
  4. Receive achievement.
