After drinking Elixir of Freedom, the effect is almost entirely lost after drinking and obtaining the “Cooling Down” status effect. Instead of +21 Stamina it changes permanently to +6 Stamina. However the tool tip still shows the +21 effect.

This is not a new bug, it is a very, very old bug.
The max thirst/hunger bonuses will overwrite normal +health/stamina buffs, then when those other buffs wear off, your max health/stamina will go negative.
Has already been reported when 3.0 was released. Funcom did not care to fix it.
It was reported before 3.0 was released too.
This bug was around long before 3.0, it just wasn’t very noticeable before then because there were very few items that directly increased health/stamina instead of attributes.
One of the deep sea fish originally gave bonus health, as well as the Potion of Lost Souls, but they “fixed” the bug on those by removing that bonus entirely. By the time 3.0 launched, the Elixir of Freedom was the only item left that increased health or stamina, and it was just as bugged then as it is now.
Single Player, Happens on both Exiled Lands and Siptah.
