A recipe does exist in the game files, but it is currently unobtainable.
Most of the wiki is auto-generated by a bot that datamines the devkit. The bot does not know if items or recipes are fully implemented or not.

In Wiki you can even find a wearable Witch Queen mask which is and always was obtainable via Admin panel only. There are lots of items in Devkit that are abandoned by Funcom or even added on purpose for modders and server admins.

However, I said it once and I will say it once more!!!
If players that play Siptah “think” that they unlocked Siptah they are really wrong, extremely wrong actually! Siptah Surges still hide a lot secrets, but rng can make you believe that you’ve seen it all!
Siptah has more secrets! I even believe that the dreger is hiding being a lower surge but people mostly prefer the higher Surges.
After the official release of Siptah it is really rare and random for someone to do a surge lower than tier 4.
