I’ve seen this bug before, and I thought it was fixed for awhile, but it seems to have shown up again.
Sometimes when you pick up a mission item what will be automatically added to your mission item hot bar, it adds ‘two’ items to the bar.
It’s especially annoying when you are trying to do ‘Red Handed’.
