If you are in a region that allows full nudity (ESRB = partial nudity. PEGI/USK = full nudity) then setting it to “full” in both server settings and game settings should do the trick.
If that’s not workin for ya and you confirm you’ve done both, lemme know.
I’m playing in Italy on PS4, USK/PEGI recognized buy not legally mandatory (on wiki it does not clarify much) in our country.
But I have to say that 2 or 3 patches ago (not sure how many) I had no problems about that, now nor the partial or the complete nudity are working, currently I have the 1.19 version dowloaded and installed
I’m playing in France on Ps4 and Nudity’s gone with the recent patch.

Hi Nicole, the settings DO save on the(in game menu) , they reset on the (Main menu) ,but it doesn’t matter because their still saved on the in game menu, includeding nudity, the only setting that doesn’t save is (Show contextual controls) PS4 tested on singleplayer only 
Ok I resolved in the “normal way”.
The problem was (I think) that I wasn’t giving the setting the proper time to “load” and set.
If I can be of any help I recommend to change nudity on the setting panel >server setting general (scroll to bottom and find the command) and THEN set it ALSO in setting panel (gameplay).
In this order it should be work properly, maybe I was doing wrong and too fast before.
