If you return an item to inventory, a table for example, any items sitting on the table will vanish.

I lost a candle 
This is not a bug it’s a game feature to prevent left over deco everywhere. You will have to pick everything from the top down if you don’t want to loose anything
They could have decor items fall into a loot bag instead of simply vanishing.
Indeed but they didn’t, everything vanish if not picked. Also be careful when setting wheels of pain an or altars without foundations. If there is a stone or branch on the floor and you don’t see it, it will make the item disappear also. Also they will randomly disappear if not set on foundations after a server restart
This seems to only be the case if decay is enabled. This happened to me once before I disabled decay.
But yeah it’s definitely affecting a lot of people on official and private dedicated servers.
