This is part of fixing how they screwed up combat on PC in the last chapter - sounds like they’ve found a way to cause the exact same thing for you guys. Hopefully either turning this setting on (or off if it’s on) should fix it for controller.

Fatalities - I believe there is supposed to be a setting somewhere (maybe under ‘Server settings’) to turn them on or off, if that’s what you mean. Otherwise it should just be ‘some enemies’ go into a standing and wobbling animation when they reach zero HP - you then need to wait to ensure you don’t continue a combo (but not too long or they will just fall over) and then hit attack again to execute the fatality.

wasn’t mentioned in the patch notes and Funcom ‘never’ misses big changes out of the patchnotes, so it must just be your imagination
… They also didn’t mention removing the custom sort inventory option, or the fact that they’ve made radial menus no longer use the mouse pointer - so clearly those are things that also aren’t happening.
(Hopefully my answers to the first two questions are a bit more useful than the third…)
