Perk could make just the initial abyssal call summon permanent and have chance to spawn in other temporary abyssal call on chance as it is now. You can only have one original Abyssal call and the others that have a chance to spawn in are temporary.
Also another enhancement is that while the extra spawns are uncontrolled they should be drawn to attack any target that has the bleed damage debuff so their targets are a little less random, all in all this makes demon lord much more competitive with well trained and makes melee damage with daggers make more sense. They need change demon lord to be a true demon lord the perk shall be active with 2 big demon following you all time the demons shall be like the demons from the traitor bridge event in exiles the 2 big guys with hammers the demons shall be like a permant Thrall until you die or he die the demon shall be summoned after you kill a enemy and no after landing a hit and the demons shall no target your allys.
I would prefer the Blood moon beast rather then the two with hammers, the blood moon beast has blood bolts, acid spit, spawns adds to help, and blood ball for aoe.
But the bmb is over the top and they would nerf it to a follower level. Do you trust that? The 2x demonic soldiers is actually a nice touch but I personally think they need to boost the current demonic followers that can be summoned because those models are gorgeous but due to how they are glorified very temporary mid tier pets, they aren’t that popular.
I like the idea of adding permanence to these summoned creatures as the perk plus the ability to to summon temporary ones.
So how about the perk is two of your summoned creatures are now permanently there until you summon something else (one mount and one demon) plus chance to temp summon helper demons but I would have it based on getting hit vs attacking.
The problem summoning temporary followers are you can lag the server think about 5 players using It in the same time i think the permant followers Will be best since It cant lag anyone.
The original Abyssal call has the ability / chance to spawn additional temporary demons like it but they also despawn after 5 mins or so, this is ok as the uncontrolled ones are drawn to attack something with bleed damage debuff.

Your mastery of the flows of corruptive influences in sorcery is to the point where you always have a Summoned Demon at your side that does not count in your follower limit. It functions and levels like a normal Summoned Demon but without any timer associated with it. If it dies, a new one is immediately summoned in a ball of Orange magic (the summon animation) and immediately follows you. It will ALWAYS follow you and you cannot put it on scouting or remove it until you remove the perk. Once the perk is removed, it disappears.
