Occasionally, the ‘Take all’ does not have [Spacebar] below it. When that happens and I do not notice it, hitting the spacebar drops the first thing in my inventory. Not gamebreaking, since it’s easy to see nothing was looted. Click the ‘Take all’ button, close the box with escape (dammit 
) and retrieve the dropped item from the dropbag.
Here is the good and the bad (and the ugly?)
No common denominator is present. Sometimes it lasts an entire gaming session; often it occurs once or twice. Location does not matter. It’s also been present in a solo, personal computer game. I don’t remember if it has happened with containers, though.
