The executioner disappeared and will not spawn again

The executioner disappeared with 5% hp and never spawned again

Bug Description:

Was farming the executioner boss, mid fight he disappeared and never spawned again, spent about 1.5hours looking for him, all other mobs in the came spanwed and respawned but the boss does not.

I am thinking he is stuck in the mesh, only a server restart can bring him back.
