So i just finally managed to get freya after she was bugged for 2 days for me as she didnt want to do her last quest but now i finally got her her face went from freya to ingrid she looks completely different

Probably, the shocks she experienced left an imprint on her appearance and changed it beyond recognition. 
In general, she began to look like a transgender person.

I just caught Lianeele.
And imo it is still the same pattern - the thralls look in some usual normal way before you convert them, but after the conversion they usually have completely randomized face proportions. Apparently it is the same with the new unique thralls as well.
And your Freya has the same gapped tiny piggy eyes as my last Lianeele did.
Absolutely no reason for this tragedy
