Upgrade Altar of Yog to Abyss gives Abyss in altar inventory instead of upgrading

We upgraded our altar and got the item instead of altar being upgraded.

Steps on how to reproduce issue:

1. Upgrade Altar of Yog to Abyss
2. Wait (may have been during crash)
3. See placeable Abyss altar item in altar inventory.

I just performed this action last night on officials, and did not have the same experience.

I have screenshots from Official #1744. The server did crash shortly after so it may have been crafting when the server went down.

I had this happen on testlive upgrading altar of Mitra, and reported it there.
