Water surfacing bug killed me and then for some reason my horse who was above water

Basic Info:

Platform: PlayStation 5
Issue Type: Gameplay
Game Mode: Online Official
Server Type: PvE
Map: Exiled Lands
Server Name: 3181

Bug Description:

My horse was in water and wouldn’t exit it (their head was above the water) I died in the water because my character kept falling to the lake bed as though there was no buoyancy and instead normal gravity (I’d take damage upon falling and hitting the lake’s bed) my character then drowned and at the same time, my horse died for no reason (lost weapons and it’s saddle). My thrall standing outside the water did not die. Pretty frustrating bug…

Bug Reproduction:

I’m not sure how to repro it, it was at the lake south of ice spire cavern. I fell into the water (I had no cold damage when initially trying to climb out so it weren’t damage stopping me from exiting) then couldn’t exit when trying to surface, I just kept falling to the lake bed and my oxygen didn’t refresh upon surfacing several times. My horse I was trying to move to a new spot, it said when I died he was no longer following me, then upon respawning and getting there, I noticed in the event log the horse suddenly died

At the red circle north of the ice lake, there’s a little hole in the ice close to a pointed rock, it’s the only small hole in the ice in that spot I’ll try and get a closer picture)

This is not a bug due to any location. It occurs when reaching the surface and trying to climb out. Very easy to replicate in the frozen lake near Bridge of Betrayer. When you try climbing onto an ice ledge your character will immediately fall. If the fall damage doesn’t kill, lack of air will
