When she became my friend, I put some medium armor on her. Then I noticed how squishy she was and put my armor on her. (Of course the game crashed…) When we got to her new home I removed all my armor and put her original armor on.
I’ve come and gone several times but this time I noticed she was wearing my helmet, which is on my head, not in her inventory. It’s an illusion I guess but I’d rather look at her pretty face.
So I equipped her with a Relic Hunter Turban. Removed it, exited and reloaded. She was wearing my helmet.
Then I reequipped the Relic Hunter Turban. Exited and reloaded. She’s wearing the turban as expected. So I removed it, exited and reloaded… Now she’s wearing an illusory turban.
this happens sometimes after placing a thrall with armor on and then removing it. The work around is to remove the armor, then have the thrall follow you a little distance and then place her again. she will retain the look she has after placing. if the armor is showing but not on her, put something on and remove it to clear the illusion, and then do the follow/place thing again. I see this a fair amount while playing on official servers.
Bugs been around for a year and a half. I’ve had bearers I’ve taken the pack off of still display with it on.
I’ve had this issue; although not as bad now as it used to be, on official, privet and single player.

On our private server we are planning on giving all our girls invisible Commander helmets for the poison resistance never known when we might want to let loose some arrows.
Same issue here! Sadly I unfriended this guy and cannot show any images, but I gave him a pict armor and when I quit it from his inventory he stayed with the helmet for days. I couldn’t fix it… since he was a low level friend, as I said, I unfriended him.
Put her in follow mode. Teleport to a different biome or area far enough away from your base that it doesn’t render. Put the outfit on you want her to wear. Teleport back to base. Put her back in her spot.
Had to load an old file (I save backups) and tried to follow my same steps. I’m pretty sure I was successful because I made the game crash (sent crash report) and when I reloaded she was wearing an illusory helm.
