Also notice that some rooms are missing NPC, I have stand and wait for 5 minutes but nothing spawn.
Maybe other vaults get the same situation.

Please repeat the steps without mods to confirm.

This happens to me quite often on the official 6308 server.
When I arrive at the boss room, the sound is changed, but the boss is not there.
To resolve, I have to leave the dungeon, enter again, walk to the boss’s room to find him.
What I notice is that in some points the NPCs are able to pass through walls, and when walking through the floors close to the Boss’s room I end up starting it. Then he tries to find me, enters these points on the wall, and is inaccessible.
