Is anyone having this issue on their server? All respawns are always every 15 minutes and have been for years. He is not only not respawning within the time but he is spawning as a fighter III instead of his named version.

Berserkers spawn every 15 min in official servers, every 5 minutes in single player and Xmin on private servers. This X changes with the parameters of each admin. So if you play on official servers, you have to give more info and finally report it on Zendesk if necessary.
For private servers speak with the admin

No. It is official. Somehow the bug fixed itself but after the level III spawned in its place it took 35 minutes for the berserker to respawn. After that it started working again. First time that happened in years

I know I have come to depend on those beserker spawns. They are great guard thralls through end of game content.
