Region: north America ,/Canada
Was 1st encountered when leaving volcano up path from well entering into the loading screen door area to dragonmouth. Thought nothing really of it but went back to retrieve items off fallen corpse and upon entering volcano thru dragonmouth, immediately after loading screen character died by same means.
Steps on how to reproduce issue:
1.go to dragonmouth passage from north load into volcano
2. Leave volcano thru dragonmouth passage
