[Fully leveled alter with 500 zeal, decayed. Not the walls, foundations, or any other structure in or around the base. Built months ago on foundations]

Could you please share further information, preferably through a screenshot, of the spot where the altar had been placed prior to the decay?

It is happen to me also. Jhebal sag altar lvl 4 with over 550 zeal simply vanish. An archpriest was set on it.
Biggest no no on PVP!!! Just saying. Arch doesn’t come out until needed.
But as far as your problem, not seeing where an altar would even fit based on you pics.
I have set alters on and off foundations. Never lost one some times difficult to place. Looks like everyone has a different experience with various Glitches. Good luck to you all.
