Are you playing in Exiled Lands or on Isle of Siptah?

In Exiled Lands, you can get it by killing Relic Hunter minibosses in the Unnamed City.

I don’t know about Isle of Siptah, though. It’s been months since I stopped playing that map.

On the bottom left side of the isle of dusk up on a small cliff with a broken elevator on it is a camp with no name

Should be on that southernmost peninsula, laying on the alter at the top.
