Randomly crash when enter level

Bug Description:

While playing if I use low settings it will crash sometimes but ultra settings always crash when entering the level.

Crash:

In player.log and player-prev.log it says something like
Failed to create agent because it is not close enough to the NavMesh

Steps to Reproduce:

Step 1: Open the game
Step 2: enter level
Step 3: game crash (ultra settings) or playable for a while then crash (low setting)
