I have only seen this bug reported without the official template and thus disregarded. Hopefully, this meets the standards:

Bug Description:
A T4 crafter, Phalia the Blacksmith’s spawn at The Den has been removed along with the al-Merayah, and I believe the former was by accident. Would it be possible to restore the former with some hotfix, considering how few choices there are?
Bug Reproduction:
The NPC spawn is missing despite hundreds of visits to the Den, thus safe to assume the 1% spawn rate doesn’t work anymore, as confirmed by reports of others.

Phalia do spawn rarely at the den. Alongside the other blacksmiths
There are 3 locations one in front of the hyenna a spot that can Spawn any of the dogs crafters , and another one at the North east entrance next to bearers and another camp at south east .
Yes, there is a very very small chance of her spawning at one of the randomized thrall spawns, but there used to be two blacksmith spawns where she had a more reasonable spawn chance.
One of them was at the Ruins of al-Merayah, which was wiped for the fortress, the other was at that location shown above in the Den, which was deleted around the same time when thrall spawns were adjusted.
This issue was reported multiple times since then.
