Game mode: offline Single player Ps4

3 times now after death my body disappeared and I cant get my loot back. Seems to be random when it happens its not every time. Happened in single player offline and playing coop with a friend. Also had a pet disappear once, he showed on the map where he was but he was invisible. I was able to rescue him through the followers tab but I lost all the loot that he had on him. This all seems to be happening since the last update.
Another Bug there seems to be an issue with the Iron truncheon as I can easily knock out anything in my path in one hit.
Steps on how to reproduce issue:
- Happens after death But random
- Single player offline and coop
